Together for Health

Together for Health is a new project, funded by the Department of Health's Innovation and Excellence Fund, working with Age UKs in North Yorkshire,
Bradford, Barnsley and Wakefield.

The aim of the project is to support older people whose health is being affected by their social isolation and loneliness to connect back into their local community.  We want to evidence whether, with the support of the voluntary sector, we can reduce loneliness and isolation amongst older people, and therefore reduce their likelihood to
use health resources through reduced hospital admissions, A&E presentations, GP presentations and so on.

Together for Health will work with patients to:

· Set personal health and social goals

· Create networks of peer and social support

· Maintain and develop healthy lifestyles

· Reduce isolation and improve wellbeing

We will start with an assessment of their needs, and use a goal setting technique which will be reviewed. During their involvement with the project older people could be referred into the following Age UK services:
 

 

The referral criteria to the project is as follows:

· Isolated or lonely –  risk factors are live alone, recently bereaved or divorced, prone to falling, family lives far away, becoming a carer or giving up caring, retirement

AND

· Health resource usage in the last 12 months – 'frequent flyer' at GP or A&E, and / or hospital admission in the last 12 months
